Female to male (FRM) top surgery is provided by the best in class board certified plastic surgeon, Dr. Keith Blechman. Top surgery is a gender reassignment procedure for transsexual men in order to create a masculine chest.  Top surgery involves removing the breast (referred to as subcutaneous mastectomy), and depending upon the patient’s anatomy, may also include free nipple-areola grafts, nipple-areola resizing and reposition, and/or liposuction.  Top surgery is the most commonly performed gender reassignment surgery for transsexual men, allowing patients to live more comfortably in their preferred gender role, thereby improving psychological and social functioning.  Top surgery may be the only surgical procedure he takes in his transition.

During your consultation Dr. Blechman will discuss your desires and expectations with you.  Measurements will be taken and an individualized plan tailored to your needs will be explained.

There are a variety of top surgery techniques available.  Depending upon the size and shape of your breasts, and the elasticity of your skin, Dr. Blechman will counsel you on what technique is appropriate for you.  Some patients, usually with smaller breasts, are candidates for an incision only around the areola (sometimes referred to as a keyhole technique), at which time the areola can often be resized.  Patients with larger breasts may require incisions across the breast (sometimes referred to as a double incision technique), leaving a scar along lower border of the pectoralis muscle.  The double incision technique generally involves using a free nipple-areola graft.  Regardless of what surgical approach is used, liposuction is frequently used to help refine the new male chest contour.  The procedure is performed under general anesthesia and takes approximately four hours.  Patients return home that same day.

Bruising and swelling are normal and may take a few weeks to resolve.  Most of the stitches are beneath the skin and do not need to be removed.  If free nipple-areola grafts were placed, gauze bolsters are sewn over the grafts and to the skin.  These bolsters are removed within a week.  Surgical drains may be used which will be removed in the office a few days after surgery.  Patients are permitted to walk and return to their activities of daily living immediately following surgery, however strenuous exercise should be avoided.  Patients typically return to work in about a week.  An elastic garment will be placed over the chest to provide snug compression, and it must be worn continuously for approximately one month.  It may take many months before all the swelling resolves and the final results are seen.

Complications are uncommon.  As an experienced, board-certified plastic surgeon, Dr. Blechman takes a conservative and educated approach to top surgery, and will discuss in detail all the risks, alternatives, and benefits prior to performing surgery.
